Bridge from Below only works while it's in your graveyard. If a nontoken creature is put into your graveyard from the battlefield while Bridge from Below is there, it can make a 2/2 black Zombie token. If any creature is put into an opponent's graveyard from the battlefield while it's in your graveyard, it will be exiled. If both kinds of triggers happen at once, you choose the order they resolve.

Bridge from Below does nothing while it's on the battlefield. Both of its abilities are triggered abilities that only trigger when Bridge from Below is in your graveyard. First ability: Whenever a nontoken creature is put into your graveyard from the battlefield, if Bridge from Below is in your graveyard at the moment that event happens, that ability triggers. When that triggered ability resolves, it creates a 2/2 black Zombie creature token. If Bridge from Below is not in your graveyard at the moment the creature is put into the graveyard, the ability does not trigger. Also, if it triggered but Bridge from Below is removed from your graveyard before the triggered ability resolves, the token is not created. Second ability: Whenever any creature is put into an opponent’s graveyard from the battlefield, if Bridge from Below is in your graveyard at that moment, that ability triggers. When that triggered ability resolves, exile Bridge from Below from your graveyard. If multiple creatures cause both kinds of triggers at the same time, you (the controller of Bridge from Below) choose the order in which its triggered abilities resolve; you can therefore choose to let the token-creating ability resolve before the exile ability.

Rulings

While Bridge from Below is on the battlefield, it has no effect. It does something only if it's in a graveyard. Yes, this is a bit weird.

If a nontoken creature you control and a creature an opponent controls die at the same time, you choose the order in which Bridge from Below's triggered abilities resolve. You can create a token before you exile Bridge from Below.

Neither ability cares who controlled the creature that died, only which graveyard it was put into. This means that if an opponent controls a creature you own, its death causes Bridge from Below's first ability to trigger, not its second.

If Bridge from Below isn't in your graveyard immediately before a creature dies, its abilities won't trigger at all. If Bridge from Below isn't still in your graveyard as its first ability resolves, you won't create a token.
